Nominations are called from eligible members for the election of 6 Member Directors.
Nomination and Proxy Forms are available from any South West Aboriginal Medical Service Clinic or Administration Office.
Important:
All nomination and proxy forms must be returned directly to the Returning Officer.
SWAMS staff cannot accept or submit forms on your behalf.
